By submitting a referral for services, you expressly consent to having Elements of Motivation, our providers, and intake team contact you about services, by text message, phone, or email to the telephone number/email you have provided.
This consent applies even if that telephone number is on a corporate, state, or national Do Not Call Registry.
Please note, our staff will attempt to contact you using all methods for communication listed, to avoid delay in initiation of services. We understand that there are a high number of spam calls. Please be on the lookout for communication with our staff and providers so that we may schedule with you as soon as possible.
For the soonest available appointment, please indicate "Telehealth is okay" on the referral form. There is a waitlist for both telehealth and in-office, but it may be a longer waitlist for in-office therapy services.
